Thyroid nodule radiofrequency ablation
An outpatient procedure that shrinks benign thyroid nodules without surgery, without a scar and without general anesthesia — preserving the thyroid gland and its function.
Book an evaluation →Radiofrequency ablation is an outpatient procedure that shrinks benign thyroid nodules without surgery, without a scar and without general anesthesia. Through a fine, ultrasound-guided needle, controlled heat is applied so the nodule gradually shrinks over the following weeks. It is indicated for selected benign nodules causing discomfort, pressure or cosmetic concern, and it preserves the thyroid gland and its function. In Costa Rica, Dr. Christian Hernández (license MED6760), a head, neck and thyroid surgeon, evaluates each case by ultrasound to determine whether you are a candidate.
What is thyroid radiofrequency ablation?
It is a minimally invasive technique that uses radiofrequency energy to generate controlled heat inside a thyroid nodule. That heat inactivates the nodule tissue, which the body gradually reabsorbs, shrinking it over time.
The entire procedure is performed under real-time ultrasound guidance, which allows the nodule to be treated precisely while protecting neighboring structures of the neck. It requires no incisions or stitches: the energy is delivered through a very thin needle. Unlike surgery, the gland is not removed, so in most cases normal thyroid function is preserved.
Am I a candidate? Who is it for?
Radiofrequency ablation is generally considered for people with thyroid nodules confirmed as benign who also present any of these situations:
- Discomfort when swallowing, or a feeling of pressure or a "lump" in the neck.
- A visible bump causing cosmetic concern.
- Progressive growth of the nodule across check-ups.
- Certain nodules that produce excess hormone, in selected cases.
- People who prefer to avoid surgery or who have higher surgical risk, when the case allows it.
Before ablation, an ultrasound evaluation is usually required and, in most cases, a biopsy confirming the nodule is benign. Every case is different: an individual evaluation determines whether this treatment is right for you.
When is surgery needed instead of ablation?
Radiofrequency ablation does not replace surgery in every case. Surgery is usually recommended when the nodule is suspicious or thyroid cancer is confirmed, when it is too large to treat this way, or when other findings require removing the gland. That is why knowing the warning signs early matters: see our article on thyroid cancer symptoms (in Spanish). The decision between ablation and surgery is always personalized.
How is the procedure performed? Step by step
- Prior evaluation: the ultrasound and biopsy are reviewed to confirm the nodule is a candidate for ablation.
- Local anesthesia: applied only to the neck area; you remain awake and comfortable.
- Ultrasound guidance: with real-time imaging, a very thin needle is advanced into the nodule.
- Energy delivery: controlled heat treats the nodule point by point, with precision.
- Observation and discharge: after a brief monitoring period, most patients go home the same day.
Advantages over traditional surgery
- No visible neck scar, since no surgical incision is required.
- It preserves the thyroid gland and, in most cases, its hormonal function. To understand why this matters, read about thyroid surgery and gland function.
- Outpatient procedure, no general anesthesia.
- Usually fast recovery, with a prompt return to routine.
It is worth remembering that no technique is ideal for every patient. Ablation is an excellent option in selected cases, and medical evaluation determines when it offers the greatest benefit.
Recovery and results
Most people return to their activities within a short time. Mild discomfort or neck tenderness in the first days is normal. The nodule does not disappear immediately: it shrinks gradually over weeks and months as the body reabsorbs the treated tissue.
The degree of shrinkage varies with each nodule's size and characteristics, so results are individual. The specialist schedules follow-up ultrasounds to track progress.
Is the procedure safe?
Performed by an experienced specialist under ultrasound guidance, radiofrequency ablation is considered a safe procedure. Like any medical act, it is not risk-free: temporary discomfort, a small bruise or, infrequently, temporary voice changes can occur. Experience with neck anatomy and knowledge of nearby structures —such as the nerve that controls the voice— are key to performing the procedure as safely as possible.
